Terms of Service

1. Acceptance of these terms

By creating an Ann Arbor Appeals account, checking the consent box at signup, or using the service, you agree to these Terms of Service and to our HIPAA Notice of Privacy Practices. If you do not agree, do not use the service.

2. What Ann Arbor Appeals is — and is not

Ann Arbor Appeals helps you organize documents, understand insurance denials, and prepare and track appeals. Ann Arbor Appeals is not a law firm, an insurance company, or a healthcare provider. Nothing on the platform — including output from our AI companion, Alvin — is legal, medical, or financial advice. For advice about your specific situation, consult a licensed professional.

3. Your account

• You must be at least 18 years old and provide accurate information.
• You are responsible for safeguarding your credentials. Multi-factor authentication is required for all accounts.
• You may use the service only for your own appeals or, as an authorized advocate, for patients who have designated you.

4. Your content

You retain ownership of the documents and information you upload. You grant us a limited license to store, process, and transmit that content solely to provide the service — including AI analysis you have consented to — and as described in the Notice of Privacy Practices. You are responsible for having the right to upload what you upload.

5. AI companion (Alvin)

Alvin uses artificial intelligence to read your documents and conversation history and to help draft appeal materials. AI output can be wrong or incomplete. You must review everything before relying on it or submitting it to an insurer. We log AI interactions for security, audit, and quality purposes as described in the Notice of Privacy Practices.

6. Acceptable use

You agree not to misuse the service — including attempting to access other users' information, uploading malicious code, scraping the platform, misrepresenting your identity, or using the service for any unlawful purpose. We may suspend accounts that put other users' data at risk.

7. Legal documents and consent records

When you accept these terms, we record the exact version you agreed to. Every version of these terms and of the Notice of Privacy Practices that you consented to remains available for download from your account in perpetuity, even after the document is updated. When we make material changes, we will publish a new version with a new effective date and ask for your renewed consent where required.

8. Termination

You may close your account at any time. Closing your account does not delete audit records, consent records, or other records we are required to retain by law. We may suspend or terminate accounts that violate these terms.

9. Fees and payment

The contingent arrangement — "no fee unless your appeal wins" — applies only to the contingency-fee appeal service, not to other offerings. 10. Disclaimers and limitation of liability

The service is provided "as is" without warranties of any kind, express or implied. Ann Arbor Appeals does not guarantee any appeal outcome. To the maximum extent permitted by law, Ann Arbor Appeals's total liability for any claim arising out of the service is limited to the amount you paid us in the twelve months before the claim arose. Nothing in these terms limits liability that cannot be limited by law.

12. Changes to these terms
